Background: Neonatal sepsis is one of the leading causes mortality in developing countries because its non-specific presentation. Blood culture gold standard for diagnosing sepsis, but results take 48 – 72 hours. Therefore, there a need an indicator that could be used as simple parameter to indicate possibility evolving clinician. The efficacy mean neutrophil volume must evaluated. Materials and methods: study aims evaluate utility early diagnosis neonatal sepsis. Following ethical clearance, involved analyzing from peripheral smears 50 newborns clinically diagnosed with control group included findings normal newborns. Maternal or fetal factors like pregnancy-induced hypertension birth asphyxia cause changes neutrophils were excluded. value was calculated after establishing diameter 100 per case using National Institutes Health Image J software. Results: average found significantly increased (181x104fl) than (739x103fl). A statistically significant difference (p= < 0.0001) observed.
